In this report, we present the case of an immobilized 54-year-old female with SpA who complained of a painful swollen left leg. Laboratory workup and imaging study supported the diagnosis of SpA complicated with deep vein thrombosis (DVT). The patient was given glucocorticoid, an anticoagulant, and medical rehabilitation. Symptoms improved following treatment. SpA increases the inflammatory state, leading to the increased risk of DVT. Holistic and comprehensive treatment is required in SpA patients with DVT to prevent further exacerbation or poorer prognosis.&lt;/p&gt;
